If your insurance is not listed above, your plan may include benefits for out-of-network providers. Please give them a call to find out.
Because this is a small office we do not have the capacity to check benefits for you. To learn about your acupuncture benefits, you can call the number listed on the back of your insurance card. This way you will not be surprised if acupuncture is not covered and you will know how much you will be expected to pay. Questions to ask:
Is Kate (Katherine) Chilson in or out of network?
How many acupuncture visits do I get per year?
Are there specific diagnosis codes that coverage is limited to? (Some plans only cover low back pain for example).
Is a referal from an MD necessary?
How much will I owe for each visit? Is that a co-pay or co-insurance? (Co-pay is a fixed amount, co-insurance is a % of the total billed).
Do I need to meet my deductible before acupuncture is covered? If so, how much have I met out of the total this year?
If my acupuncturist bills 97140 or 97124, will you pay for it? If yes, will I need to use one of my massage or physical therapy visits? If so, how many of those do I get per year?

Please note, Craniosacral Therapy is not billable to insurance. Massage, passive stretching, gua sha and cupping are.
